Insurance
We accept most insurance plans. We also accept payment from most PPO
and indemnity plans. Please call our office to make an appointment and
we will answer any questions you may have.
Payment Options
We accept all major credit cards, ATM cards, cash, and personal
checks. For our patients' convenience we have arranged a payment plan
through a third party, which helps our patients receive their treatment
in a timely manner.
Payment Options
The State of Minnesota taxes health care
at 2% of treatment fees. This tax is
included in our fees and will not be
separately itemized.
The doctor will discuss treatment
options with you. You will have the
opportunity to ask questions then.
Actual fees will be determined following
diagnosis and treatment.
FOR INSURED PATIENTS:
We are providers for several insurance
companies. If your eligibility and
benefits can be confirmed today, your
required payment today will be between
20 and 80% of the treatment fee. You may
be asked for a larger payment if your
claim will exceed your annual limit, if
your plan coverage is different than
stated above, or if your benefits cannot
be confirmed. There are some companies
for which we do not accept assignment of
insurance claims. We will reimburse you
for any over payments once your claim is
settled. You will need to contact your
insurance company for questions
regarding your coverage or limits. You
will be responsible for any portion of
the fees that are not covered by your
insurance (excluding preferred provider
discounts). It is not our responsibility
to confirm your coverage. If your
insurance requires an in-network
provider, please confirm with your
insurance carrier that you have coverage
at this office. In the event that your
insurance company fails to pay your
claim within 60 days of submission, you
will become responsible for payment of
the full treatment fees. We will
continue to assist you in obtaining
reimbursement from your insurance
company.
FOR NON-INSURED PATIENTS:
Treatment fees are due at the time of
treatment. For emergencies and
multi-visit treatments, a 50% initial
payment is required in addition to any
emergency fees that apply.
There are several options
available to you to complete payment
today.
� Cash, Check, or Credit card.
� Payment Plan. We offer financing
through Care Credit. Six months no
interest and extended low interest plans
are available. Please inquire for more
information.
In the event that a procedure is
initiated but you do not return to our
office for completion of treatment, you
will be responsible for 50% of the
complete treatment fees for services
rendered. Incomplete treatment fees may
not be covered by your insurance
carrier. Balances that are past due over
60 days will be charged a $3.00 billing
fee per month. Any account submitted to
collections will be charged for all
collections fees, including but not
limited to court costs, legal fees, and
collection agency feess.
